Nirvana Healthcare Management Services runs a network of 10 primary and specialty care locations across New Jersey, in Essex, Warren and Monmouth counties, with sites including Newark, East Orange, Hackettstown, Bloomfield and Morganville. The network provides integrated primary care, chronic disease management, preventive medicine, on-site laboratory services, behavioral health, neurology and psychiatry, vascular care, weight management and regenerative medicine. The founding practices behind it have more than 70 years of service.

Beyond the clinics, the company operates the Community Health Outreach Program (CHOP), which has delivered integrated care in patients' homes since 2002. The programme serves seniors and people with mental illness or substance-use disorders in private homes, senior residences, group homes, adult-daycare facilities and behavioral health centers, and its population health department received a $500,000 grant from the New Jersey Department of Health.

Care teams work with electronic health records and population health management tools. Ninety-five per cent of staff are bilingual in Spanish, and the Martti virtual translation assistant is used to support patient communication alongside bilingual colleagues. The organisation describes an emphasis on preventive care, early risk detection and coordinated treatment, a focus on access for underserved populations, and clinical environments designed to feel home-like.
