Mosaic Community Health is a nonprofit Federally Qualified Health Center that provides integrated medical, dental, behavioral health, pharmacy, and nutrition care through a network of 16 health centers serving Central Oregon, with locations including Bend, Redmond, Madras, Prineville and Sisters. Founded in 2002 following a tragic loss in Prineville that prompted community action, the organisation employs more than 450 staff and is governed by a volunteer, patient-majority Board of Directors.
Care is delivered under one roof across the network. Alongside primary care, services include school-based health centers serving children and families, on-site pharmacies where patients can collect prescriptions, and behavioral health consultants working as part of integrated care teams. All eligible clinics have earned five-star Patient Centered Primary Care Home rankings, a distinction achieved by fewer than 5% of Oregon primary clinics.
The organisation holds that healthcare is a fundamental right rather than a privilege, and states a commitment to compassionate, culturally appropriate care under the ethos "All are welcome here". Its teams are drawn to reflect the diverse communities it serves.
